In addition to accepting D-HMO patients, we also participate in a number of PPOs such as Aetna, Guardian, United Health Care, and accept most indemnity insurance plans including Delta Dental plans of all the 50 states as well as District of Columbia, Blue Cross Blue Shield. New Patients For Elective Services
You can download registration forms at SERVICE, complete them and bring them with you at your appointment. Print $25 coupon which is toward your first time treatment.
Emergency Patients
Same-day emergency treatment available, download the emergency forms below. Remember you are not our patient until we have completed a full evaluation of your dental needs but by completing the forms before you visit our office, we will give you $15.00 coupon toward your emergency sevice. You must bring the forms with you.
Payment and Financing of Your Dental Care at Our Practice
At our office, payments are prospectively paid. We accept cash, insurance payments, credit card payments. Currently we are not accepting checks but debit cards are accepted. We accept almost all insurance plans but patients are ultimately responsible for their own accounts. For certain services, payment plan can be arranged through our health financing at Capital One or Care Credit. Forms are available at our office and approval can be received within the hour for most clients. Cash paying patients can expect to receive 5% discount for paying in cash and in full. There is a coupon available (CLICK HERE) but you do not need this coupon to receive this discount. Our goal is to assist you and to make paying for your treatment affordable and less stressful and that is why we are always looking for more financing houses and banks such as Chase Healthcare Financing are currently being considered.
